What is Transition?
a coordinated set of activities for a child with a disability that is
focused on improving the academic and functional achievement of the child with a disability
facilitates the child’s movement from school to post-school activities, including:
integrated employment (including supported employment)
continuing and adult education,
independent living, or
transition service planning should begin no later than when the student begins ninth grade or turns 16, or younger if determined appropriate by the IEP (Individualized Education Program) team, and updated annually thereafter.
HGTA IS WORKING
to improve transition services for individuals with disabilities through activities that will bridge the gap among community options, prospective employers and post-secondary institutions in the Heart of Georgia area.
