What is Transition? 

Transition is

  a coordinated set of activities for a child with a disability that is

  • results-oriented 

  • focused on improving the academic and functional achievement of the child with a disability

  •  facilitates the child’s movement from school to post-school activities, including:

    • postsecondary education

    • vocational education

    • integrated employment (including supported employment)

    • continuing and adult education,

    • adult services

    • independent living, or

    • community participation



In Georgia 

transition service planning should begin no later than when the student begins ninth grade or turns 16, or younger if determined appropriate by the IEP (Individualized Education Program) team, and updated annually thereafter.


to improve transition services for individuals with disabilities through activities that will bridge the gap among community options, prospective employers and post-secondary institutions in the Heart of Georgia area.
